Single PDF form URL in SharePoint

Hi guys,

Is there a way I could upload a PDF form to a SharePoint (2010) workspace document library and have multiple users (potentially 3000) fill out the form have all their responses saved in the same document library without having to send each individual user an invite to fill out the form from within Adobe Acrobat?

My use case is below:

  • Create a form using Microsoft Word 2010
  • Use Adobe Acrobat Pro X to convert it into a PDF
  • Convert PDF into a form and sort out all the entries names, fields etc. within Acrobat Pro X
  • Upload PDF form to a SharePoint (2010) document library
  • Copy the SharePoint URL of the PDF form and paste it into a Microsoft Word document. This document will contain a host of information and will give the reader an opportunity to provide feedback via a form. This document will be converted to a PDF and stored on a portal.
  • All responses of the PDF form to be saved in the same SharePoint document library.
  • After a certain period I will manually merge all responses and export them to a Microsoft Excel 2010 spreadsheet using Acrobat Pro X.

I want to avoid having to send indiviudal user invites as the user base is quite large and it is optional for the guys fill out the form.

Any help would be most appreciated.

Thanks in advance.

Rgds

Anthony
