pop out form with radio button

I am creating a form that is quite long some of the selection will never be used. i want that part to be hidden unless the radio button is check also another button to close the form if it's open by mistake. so when the radio button is checked the user can check all the option that want and it would be printable or emailed as it is filled out.

I am used Live-cycle Design ES2
